Introducing the Western Wellbeing Hub and other updates to mental health services at Sonder

We are pleased to announce that Sonder has been commissioned to deliver an array of new mental health services, commencing 1 July 2023.

As we embark on this transition, we must also bid farewell to a few existing services that will not continue in the new financial year.

Western Wellbeing Hub

We are excited to announce the launch of the Western Wellbeing Hub. Commencing from 1 July 2023, Sonder will deliver a comprehensive range of no-cost mental health services to support individuals living in Adelaide’s western metropolitan region.

Commissioned by Adelaide PHN and delivered in partnership with Baptist Care SA and Lutheran Care, Western Wellbeing Hub will provide services focused on empowering people to achieve their personal goals and equipping them with strategies that support the self-management of symptoms.

The Western Wellbeing Hub will be based at Sonder’s Port Adelaide centre and is set to provide services for a broad range of people, including children and their families, adults and new or expecting parents. Services available will include counselling, peer coaching and navigation, psychological therapy, suicide prevention, clinical care coordination, and parenting groups.

Head to Health

Adelaide PHN has announced Sonder as the preferred provider to deliver the Head to Health Northern Centre, subject to successful contract negotiations.

The centre will provide a welcoming, low-stigma, soft entry point to support people who may be experiencing distress, crisis, or mental health conditions that are too complex for many existing primary care services. It will also include people who are ineligible or who need more timely care than that available from state public community mental health services.

The centre will also offer immediate, short-and-medium-term support and service navigation to connect people to ongoing services. It will assist people in times of crisis or as needs emerge to have access to on-the-spot care, advice, and support without needing a prior appointment.

Sonder is expected to commence delivery of the Head to Health Northern Centre service from the first half of the 2023-24 financial year pending successful contract negotiation and execution.

Northern Perinatal and Family Wellbeing

Following a recommissioning process, Adelaide PHN has announced that Lives Lived Well has been selected as the preferred provider to deliver specialist perinatal and child-focused mental health services in Adelaide’s northern region. The new provider will commence service delivery from 1 July 2023 with referrals open from 1 June 2023.

Consequently, Sonder’s northern Perinatal Wellbeing and Family Wellbeing programs will conclude on 30 June, and we are unable to accept new referrals.

However, we continue to deliver perinatal and child-focused mental health services in Adelaide’s western region, through the Western Wellbeing Hub.

Living Well with Serious Illness

Since 2011, Sonder has proudly delivered the Living Well with Serious Illness program. Commissioned by Country SA PHN, the program offers care coordination and psychological therapy services to individuals in the Gawler-Barossa region who are living with serious, progressive and life-limiting illness.

Regrettably, ongoing funding for this program has not been secured, and it will conclude on 30 June 2023. As a result, we are unable to accept new referrals for this program. We encourage referrers and community members seeking alternative service options to explore the Country SA Service Directory, available at servicesdirectory.org.au, which provides a database of services in country South Australia.

emerge

Sonder has supported young people experiencing complex mental health conditions through our emerge program since 2018, commissioned by Adelaide PHN.

Earlier this year, Adelaide PHN announced that the emerge program would be replaced by a new model called Youth Enhanced Services (YES).

Following a recommissioning process, Adelaide PHN has announced that Youturn Youth Support has been selected as the preferred provider to deliver YES.

Consequently, Sonder’s emerge program will conclude on 30 June 2023, and we will no longer be accepting new referrals.
