Toward Home Care Coordinator

Part time position available at Sonder Marion

We are seeking a Care Coordinator to join the Toward Home Wellbeing team at Sonder!

In this role, you will be responsible for working collaboratively with individuals experiencing homelessness, linking them with health services and managing crisis when it occurs.

Care Coordinators work with individuals to maintain motivation and engagement with services through management of complex health and housing needs.

This exciting and rewarding role includes providing an assertive outreach response to people sleeping rough and being part of the Toward Home Resolve assertive outreach roster.

If you have experience with working in the homelessness sector, with Aboriginal and Torres Strait Islander communities and have a good understanding of the local health system and chronic disease management, we encourage you to apply.

Part time position available at headspace Edinburgh North

We are seeking an Employment Specialist to join the headspace Edinburgh North team and assist young people who volunteer for the program to find and retain competitive employment.

The Employment Specialist will facilitate intensive and individualised support. With a case load at a maximum of 20 active clients, you will have the freedom and flexibility to support young people in a way that is currently rare within the vocational sector.

Your responsibilities will include, but are not limited to, facilitating intake, engagement, assessment, job placement, coaching and post placement follow-along support.

The right person for this role will have a minimum of 3 years experience in employment services, vocational rehabilitation, career development or a related field and experience in working with young people, families, agencies and communities.

We are seeking volunteers to support us in providing meaningful input, feedback and advice on Sonder’s youth mental health services!

Responding to young peoples’ needs should be undertaken in the context of significant people in their lives, whether they are family members, carers, partners, or friends. This Advisory Group underlines Sonder’s commitment to being an appropriate and acceptable service to young people and their families and friends.

Volunteers will undertake a range of responsibilities including representing family and friends on relevant headspace centre consortiums and advising on policies, plans and projects.

Mental Health Clinician

Our Shared Care with GPs service provides support on the path of recovery by delivering evidenced-based clinical therapy as well as working collaboratively with consumers and their GP’s to monitor health needs. Learn more about Sonder’s Shared Care with GPs program here.

This exciting program offers recovery-focused clinicians the opportunity to foster personal recovery through a focus on strengths, values and goal striving; creating increased hopefulness, empowerment, quality of life and wellbeing. Sonder is able to offer a tailored appointment schedule of 4 sessions a day, offering the time and space to engage both clients and their support networks.

We are looking for accredited mental health professionals with knowledge, skills and specialist experience in delivering evidenced-based, focused psychological intervention to people diagnosed with a Tier 3 low prevalent, chronic or complex mental health disorder.
