Looking for help to manage your chronic medical condition?

The Integrated Primary Care program supports people living with chronic medical conditions to access allied health services that enable them to better manage their health.

Our Care Coordinator will work in consultation with your GP to advocate for your health needs and arrange a team-based approach to your care.

We will support you to access our network of allied health providers including physiotherapists, dietitians, diabetic educators, exercise physiologists and podiatrists.

Together with your GP, we will find ways to help you manage your chronic medical conditions and improve your wellbeing.
