Booked Psychiatric Assessment

Psychiatric assessments provided in consultation with a Psychiatrist, GP, and Senior Mental Health Clinician.

GPs can access a Booked Psychiatric Assessment for patients either as part of, or independent of a Mental Health Treatment Plan.

The psychiatric case conference team includes the patient, their GP, a psychiatrist and Senior Mental Health Clinician. The conferences are held at the treating GP’s medical clinic.

This service is available for patients living in the council areas of Playford, Salisbury or Tea Tree Gully.

Appointments generally last an hour and there is no fee associated.

To arrange an assessment, a Booked Psychiatrist Assessment Referral Form must be completed by the patient’s GP and faxed to Sonder on (08) 8252 9433.

Upon receipt of the referral, Sonder will arrange the appointment involving the Psychiatrist, Patient, GP and Senior Mental Health Clinician.
