Supporting people living with chronic medical conditions to access free allied health services
Make an enquiryThe Integrated Primary Care program supports people living with chronic medical conditions to access allied health services that enable them to better manage their health.
Our Care Coordinator will work in consultation with your GP to advocate for your health needs and arrange a team-based approach to your care.
We will support you to access our network of allied health providers including physiotherapists, dietitians, diabetic educators, exercise physiologists, podiatrists and respirator educators.
Together with your GP, we will find ways to help you manage your chronic medical conditions and improve your wellbeing.
Allied health services are provided by health professionals who are not doctors or dentists.
They provide specialised support in preventing, diagnosing and treating a range of conditions and illnesses, and help you to manage your physical and mental health.

We will put you in touch with a local GP to access a referral to the program. If the GP finds the program is suitable for you, they will complete the Referral Form and send it to us.
Once we receive your referral, our Care Coordinator will work with you to assess your health needs and coordinate access to our network of allied health providers

Please note, that not all allied health providers are available across all regions. Some services may be provided via Telehealth.
