Pathways to Recovery

Support to transition from tertiary mental health services to services in your community.

Moving forward after hospital or specialist mental health care can feel like a big step.

When it’s time to transition back into your community, we’ll be with you every step of the way.

With decades of experience, our team provides compassionate and holistic care to help you build confidence on your journey of recovery. As you take your next steps, we’ll work together to keep you feeling strong and supported.

Getting back to everyday life can take time, and that’s okay. Our support is available for up to six months.


Important update: 

Pathways to Recovery replaces the former Mental Health Shared Care program. From 1 January 2026, referrals will only be accepted from LHN Community Mental Health Teams. GPs can continue to refer into this program until 31 December 2025.

What do we offer?

  • Care coordination

    We'll work with other services to help you get the right support and resources for your needs.

  • Psychological therapy

    Talk with a clinician in a safe space to better understand yourself, manage challenges, and work toward your recovery goals.

  • Lived experience

    Our Peer Practitioners have their own lived experience of recovery. They'll walk alongside you, offering understanding, encouragement, and practical support.

  • Integrated care

    We'll stay connected with your doctor and care team to make sure everyone understands your goals and supports you in the best way possible.

Accessing this service

Am I eligible?

Pathways to Recovery is designed for people aged 16-64 who:

  • Are living with complex and challenging mental health concerns;
  • Have recently accessed tertiary mental health care (and may be transitioning off a Community Treatment Order); and
  • Are ready to transition to a community-based, recovery-oriented program and require services to support a positive transition.

People aged 65+ may be eligible if no other appropriate supports (such as NDIS or aged care services) are available.

Who are the Pathways to Recovery Clinicians?

Pathways to Recovery is delivered by a team of clinical care planners, peer practitioners, and mental health clinicians. They work together to help you achieve your goals.

How long can I access support?

Support is available for up to six months.

How do I get started?

If you’ve been recommended for Pathways to Recovery, your Community Mental Health Team will make a referral. Our team will then contact you to arrange your first appointment and start planning your journey.

Information for referrers

Referrals are accepted from Community Mental Health Teams within NALHN and CALHN.

Before submitting a referral, please contact our team to discuss client suitability on (08) 8209 0700. To make a referral, send the following documents to Sonder via fax – (08) 8252 9433 – or email:

  • Completed Referral Form.
  • Discharge letter/summary.
  • IAR-DST (optional).
  • Client consent form.

We will confirm acceptance or decline of referrals within two business days.
