Change of process for GP referrals to Sonder’s Adelaide PHN-funded mental health programs
An update for Adelaide metropolitan-based GPs
Commencing Monday 6 July 2020, Sonder will no longer accept direct GP referrals for our Adelaide PHN-funded mental health programs servicing the northern & western suburbs of Adelaide (excluding Gawler/Barossa).
Instead, these referrals should be directed to the APHN’s Central Referral Unit
Adelaide PHN operates the Central Referral Unit (CRU) to support referrals to all of their commissioned primary mental health care, alcohol and other drug (AOD) and psychosocial services. The CRU team clinically triage referrals and refer on to commissioned service providers, such as Sonder, as appropriate.
By operating a single point of entry to services, Adelaide PHN aims to lessen the burden on the referrer and make access to a wide range of services simpler.
The CRU hold information about eligibility criteria, wait times and other relevant information to ensure people are matched with the right care based on their needs and preferences.

When will the changes be enacted?
From Monday 6 July 2020, all GP mental health referrals for Adelaide PHN-funded programs that have been sent directly to Sonder (i.e. without having gone to the CRU) will be sent back to the referrer with instructions on how to send via the CRU.
NOTE: Referrals for clients who reside in the Country SA PHN-funded Gawler/Barossa region can still be sent directly to Sonder.

Sonder’s Walk-in After-hours Mental Health Service continues to be available without the need for a referral for all clients 18 years and over with no appointment required.
Operating Thursday, Friday, Saturday & Monday from 5 pm – 10 pm at Sonder Edinburgh North.
