What does the program involve?

Tailored care plans

Your journey starts with a personalised care plan, developed in collaboration with our Care Coordinator to reflect your unique health needs and goals.

This plan serves as a roadmap, ensuring that everyone involved in your care is working together to support your wellbeing.

Holistic support to help you eat well, move more, and feel better

Build the skills you need to improve your health with our multi-disciplinary team –  including Care Coordinators, Dietitians, Wellbeing Coaches, Exercise Physiologists, Exercise Scientists, and Nutritionists.

Whether you want support with nutrition, exercise, or general wellbeing (or all of the above!), we’ll tailor your care based on your goals. You’ll receive practical advice and strategies to build healthy habits, boost your motivation, and overcome barriers to positive change.

 

Flexible support that fits your life

We understand that everyone’s needs are different — that’s why our program is designed to be flexible.

Our team will work with you to create a support plan that suits your lifestyle. Whether that’s a once-off session, regular weekly sessions, or something in between. Choose from in-person or online support, depending on what works best for you.

Accessing this program

Am I eligible for Healthy Habits?

Healthy Habits is available to adults aged 16 years and over who:

  • Are at risk of, or currently living with, a chronic condition such as: cardiovascular disease, type 2 diabetes, heart disease, weight management issues, or metabolic conditions; and
  • Live in one of the following council areas:
    • Adelaide Hills
    • Gawler
    • Barossa
    • Port Pirie
    • Port Lincoln
    • Port Augusta
    • Or surrounding areas; and
  • Identify as one or more of the following:
    • Culturally or linguistically diverse
    • Aboriginal and/or Torres Strait Islander
    • A person living with a mental health condition; or
  • Hold a Health Care Card or Concession Card.

How do I access the service?

Getting started with Healthy Habits is easy! You can refer yourself or ask a health professional to make a referral.

Once we receive your referral, we will contact you and complete a health screening before starting the program.
