Whether you’d like to focus on improving your fitness, eating habits, or overall wellbeing, we’re here to guide you with practical strategies, ongoing encouragement, and support through life’s ups and downs — so you can feel better, move more, and eat well with confidence.
Your journey starts with a personalised care plan, developed in collaboration with our Care Coordinator to reflect your unique health needs and goals.
This plan serves as a roadmap, ensuring that everyone involved in your care is working together to support your wellbeing.
Build the skills you need to improve your health with our multi-disciplinary team – including Care Coordinators, Dietitians, Wellbeing Coaches, Exercise Physiologists, Exercise Scientists, and Nutritionists.
Whether you want support with nutrition, exercise, or general wellbeing (or all of the above!), we’ll tailor your care based on your goals. You’ll receive practical advice and strategies to build healthy habits, boost your motivation, and overcome barriers to positive change.
We understand that everyone’s needs are different — that’s why our program is designed to be flexible.
Our team will work with you to create a support plan that suits your lifestyle. Whether that’s a once-off session, regular weekly sessions, or something in between. Choose from in-person or online support, depending on what works best for you.
Healthy Habits is available to adults aged 16 years and over who:
Getting started with Healthy Habits is easy! You can refer yourself or ask a health professional to make a referral.
Alternatively, download the referral form and email it back to our team.
Once we receive your referral, we will contact you and complete a health screening before starting the program.
