Healthy Habits: Program updates
Sonder’s Healthy Habits program has been updated to better support people at risk of, or living with, chronic health conditions. With a stronger focus on cultural inclusivity, and flexible delivery, the program now offers more accessible support to help individuals take control of their health and wellbeing.
What support is available?
Healthy Habits provides integrated, holistic support to help individuals build lasting exercise, nutrition, and wellbeing habits to improve their health. Services include:
- Exercise support: Tailored advice and guidance from qualified Exercise Physiologists and Exercise Scientists.
- Nutrition support: Practical and personalised nutrition advice from Dietitians and Nutritionists.
- Wellbeing coaching: Motivational health coaching to help reduce barriers and boost motivation.
Clients also receive a tailored care plan, created in collaboration with a Care Coordinator, to guide their progress and ensure all support aligns with their goals and health needs.
Who is eligible?
Healthy Habits is available to adults aged 16 years and older who:
- Are at risk of, or currently living with a chronic condition, such as:
- Cardiovascular disease
- Type 2 diabetes
- Heart disease
- Weight management issues
- Metabolic conditions; and
- Live in one of the following council areas:
- Adelaide Hills
- Gawler
- Barossa
- Port Pirie
- Port Lincoln
- Port Augusta
- Or surrounding areas; and
- Identify as culturally or linguistically diverse, Aboriginal and/or Torres Strait Islander, or a person living with a mental health condition; or
- Hold a Health Care Card or Concession Card.
People living in the Adelaide metropolitan region are not eligible for the program.
Flexible delivery
We understand that everyone’s needs are different — that’s why our program is designed to be flexible.
Our team works with participants to create a support plan that suits their lifestyle. Whether that’s a once-off session, ongoing weekly support, or something in between. Participants can also choose from in-person or online support, depending on what works best for them
How to refer
Healthy Habits aims to empower people living with or at risk of chronic illness to make lasting changes in exercise, diet, and wellbeing. If you are working with someone who meets the eligibility criteria, a referral to this program could be a valuable step towards better health.
We welcome referrals from health professionals and self-referrals via our online referral form.
