Get support to access palliative care
Learning you, or someone you love, have been diagnosed with a life-limiting illness is unimaginable. Knowing what to do next can be overwhelming.
Palliative Care Connect is a new statewide service in South Australia that provides information and connections to palliative care and bereavement support for individuals with life-limiting illnesses, and their families, friends, and carers.
Funded by the Australian Government Department of Health and Aged Care, Palliative Care Connect offers a free phone service and website to guide and support individuals in accessing the care they need.
Through the phone service, Palliative Care Navigators offer essential information, connect you to appropriate services, and empower you to make choices that align with your cultural and personal preferences.
Developed in partnership with Flinders University, the Palliative Care Connect website, (palliativecareconnect.com.au) serves as a comprehensive resource hub. It features key information about palliative care and bereavement, a service directory, and a self-assessment tool to help identify the support and resources you may need.
Sonder is proud to have been selected as the commissioned service provider to deliver the Palliative Care Navigation Service throughout regional South Australia, including the Yorke Peninsula, Mid North, and Clare Valley. To speak with a local Palliative Care Navigator or request a callback, call 1800 596 799, Monday to Friday from 8.30 am to 4.00 pm.
In addition, you can access support in-person at Sonder’s offices located in Kadina, Clare, or Port Pirie.
