Better sleep, better health: Adam’s story

Adam, a proud Aboriginal man, has always valued his independence. But living with severe sleep apnoea, heart disease, and other chronic health conditions left him feeling constantly exhausted, making it difficult to keep up with daily life. The high cost of essential medical equipment and care added to his challenges, creating barriers to managing his health and wellbeing.

That changed when Adam learned about Sonder’s Closing the Gap – Integrated Team Care (CTG ITC) program from the practice nurse at his GP clinic. With encouragement from his GP, he was referred to the program, which supports Aboriginal and/or Torres Strait Islander people in managing chronic health conditions through personalised care coordination, the development of self-management skills, and practical assistance. 

A team effort to achieve better health

Through the CTG ITC program, Adam was introduced to Caroline, a dedicated Care Coordinator, and Anthony, an Outreach Worker. Working alongside Adam, the team developed a personalised health plan tailored to his needs—starting with securing access to a CPAP machine to manage his sleep apnoea. 

To help Adam get started, Caroline arranged a CPAP machine trial through About Sleep, ensuring he had the opportunity to test the equipment and adjust to its use. Throughout the trial period, she provided hands-on support, helping him navigate the challenges of adapting to the machine. 

Recognising that cost was a major barrier, Sonder covered the cost of purchasing the CPAP machine after the trial, allowing Adam to continue treatment without financial stress. Though he initially struggled with mask leakage and discomfort, he persisted and over time, his sleep quality improved, his energy levels increased, and he felt more refreshed each day. 

The team also helped Adam access other critical health services. Recognising the impact of heart disease on oral health, Caroline arranged for Adam to receive dental care through the University of Adelaide’s Community Outreach Dental Program, which provides dental services for people who face barriers to conventional care.  

Additionally, Adam had been living with mobility issues due to an old ankle injury. Caroline organised a referral to a podiatrist, who recommended supportive medical footwear. With Sonder’s help, Adam was able to access the footwear he needed, significantly improving his mobility. 

A new lease on life

Since engaging with the CTG ITC program, Adam has experienced significant improvements in his daily life. With better sleep, he feels more awake and energised throughout the day, allowing him to better manage his work and stay active. New footwear has helped him regain mobility, enabling him to enjoy regular walks. Dental treatment has relieved pain, making it easier for him to eat comfortably and maintain a healthy diet. 

Reflecting on his journey, Adam shared “Excellent people at Sonder and great support.”

Supporting him overcome challenges, whether it was adjusting to the CPAP machine or addressing his dental and mobility needs, has been a privilege.
