Get support to take small, meaningful steps toward healthier habits. Through weekly sessions, you’ll be guided through practical skills in nutrition, movement and wellbeing with support to set goals and overcome challenges – all in a welcoming group.
This program is for individuals with, or at risk of, chronic conditions such as type 2 diabetes, cardiovascular disease, or metabolic concerns, and who also identify as:
- Aboriginal and/or Torres Strait Islander.
- Culturally diverse.
- Experiencing mental health challenges.
- Holding a healthcare/concession card.
Details
This program will run in two rounds:
- Program 1: April 15, 22, 29, 2 – 3 pm.
- Program 2: May 13, 20, 27, 2 – 3 pm.
Each session will be held at the Nuriootpa Soldiers Memorial Hall, 46 Murray Street.
