Call 1800 596 799Whether you’ve been recently diagnosed and are unsure where to begin, or your health is declining and you need help managing symptoms, we are here to assist you.
Our service offers information and connections to various supports to help you to address the practical, physical, social, emotional, and spiritual challenges that often accompany life-limiting illnesses.
Our compassionate and friendly Palliative Care Navigators are here to help you navigate available services, empowering you to make informed choices that align with your cultural background, personal needs, and preferences.
We also offer support for the people that care for you, including family and friends.
Our compassionate Palliative Care Navigators will listen to your unique concerns and develop a personalised care plan for you. They will help you access services to meet your current and future needs.
You can reach our Navigators by phone or in-person in Nuriootpa, Kadina, Clare, or Port Pirie.
You are welcome to contact the service whenever you need further assistance or have new concerns. Where possible, you can speak with the same Navigator each time.

Make a referralThe service is available to people living in the following council areas: Yorke Peninsula, Copper Coast, Barunga West, Port Pirie, Northern Areas, Wakefield, Clare and Gilbert Valleys, Light, Barossa, and Adelaide Plains.
We offer in-person appointments at our offices in Nuriootpa, Clare, Kadina, Port Pirie, and surrounding areas. In some cases, our team may also be able to visit you at home. For those who are unable to attend in-person appointments in these areas, we can provide support over the phone.
Palliative Care Regional Navigation Service is provided as part of Palliative Care Connect and supported by funding from the Australian Government Department of Health and Aged Care.
