The Healthy Eating Activity & Lifestyle (HEAL) program is an 8-week course which provides participants with life-long skills and the confidence needed to manage health conditions and maintain a healthy lifestyle.

Health conditions, such as type 2 diabetes and heart diesease, can be prevented and/or managed with support from experts and carefully considered lifestyle changes.

During the program, you will gain knowledge about your health condition and management & prevention strategies; an in-depth understanding on how you can improve your nutrition and physical activity habits; and practical skills that can be easily maintained at home.

Education and exercise sessions are held in groups, with additional one-on-one follow up sessions.

Accessing this program

Am I eligible?

HEAL acts as a prevention program, as well as a health condition management program. It is suitable for people living in country SA who fall into one or more of the categories below:

  • At-risk of developing heart disease or type 2 diabetes (including high blood pressure or high cholesterol) or type 2 diabetes
  • Need support to manage weight (overweight and obesity)
  • Have been diagnosed with type 2 diabetes, heart disease or metabolic conditions

Who are the staff?

HEAL education and exercise sessions are facilitated by Sonder’s expert team of allied health professionals, such as dietitians and exercise physiologists.

These professionals are trained to support participants who may be unfamiliar with how to make positive health and lifestyle changes or who may be unsure of how to take up physical activity.

They will work alongside your GP to help you keep on track.

How do I get involved?

To ensure there is a suitable program available in your local area, get in touch with us.

Once we have determined that there is a program available in your area, to get started with HEAL, simply book an appointment with your GP and discuss the HEAL program.

If HEAL is suitable for you, your GP will then complete the Referral Form and send it back to us.
