Looking for help to manage your chronic medical condition?

Allied Health Solutions focuses on providing allied health services to people living with a chronic medical condition, or those at risk of developing a chronic disease.

Our Care Coordinator will work in consultation with your GP to advocate for your health needs and arrange a team-based approach to your care.

We will support you to access a diverse team of Allied Health Professionals, such as Physiotherapists, Dietitians, Exercise Physiologists and Podiatrists at no cost.

We will work together to find ways to help you manage your chronic medical conditions.

This service is delivered on the Southern Yorke Peninsula.

Accessing this service

Am I eligible?

To be eligible for this program, you must:

  • Live in the Southern Yorke Peninsula region, and;
  • Be living with, or at risk of developing an ongoing chronic medical conditions that requires additional services to prevent further complications or illness.

Who are the Allied Health Solutions staff?

This service is provided by Sonder’s diverse network of Allied Health Professionals, including:

  • Diabetes Educator
  • Dietitian
  • Exercise Physiologist
  • Occupational Therapist
  • Physiotherapist
  • Podiatrist

An Allied Health Solutions Care Coordinator will work in consultation with your GP and primary health care team to support you in accessing services to treat your chronic medical condition.

How do I access this service?

To access this service, simply contact our friendly team at Sonder.

We’ll put you in touch with a  local GP to access a referral into the program.  If the GP finds the program to be suitable for you, the GP will then complete the Referral Form and send it back to us.

Our Care Coordinator will then work with your GP to coordinate initial appointments with the specialist nurse, or Allied Health Professionals identified on the referral.

The Allied Health Professionals are located in different sites across the Southern Yorke Peninsula.
