The Healthy Eating Activity & Lifestyle (HEAL) program supports people living in country SA to make long-term lifestyle changes.
Supporting people living with chronic medical conditions in the Southern Yorke Peninsula and Lower North regions of SA.

An Allied Health Solutions Care Coordinator will work in consultation with your GP and primary health care team to support you in accessing services to treat your chronic medical condition.
To access this service, simply contact our friendly team at Sonder.
We’ll put you in touch with a local GP to access a referral into the program. If the GP finds the program to be suitable for you, the GP will then complete the Referral Form and send it back to us.
Our Care Coordinator will work with your GP to coordinate initial appointments with the specialist nurse, or Allied Health Professionals identified on the referral.
The Allied Health Professionals are located in different sites across the Southern Yorke Peninsula and Lower North regions.
