Accessing this service
Am I eligible?
You are eligible for the Allied Health Solutions program if you are living in the Southern Yorke Peninsula Local Government Area (LGA) and:
are at risk of developing a chronic disease and require services to prevent future illness
or are living with a chronic medical condition and require services to prevent complications developing from their disease.
Who are the Allied Health Solutions staff?
This service is provided by Sonder’s diverse network of Allied Health Practitioners, including:
An Allied Health Solutions Care Coordinator will work in consultation with your GP and primary health care team to support you in accessing services to treat your chronic medical condition.
How do I access this service?
To access this service, simply contact our friendly team at Sonder. We’ll put you in touch with a local GP to access a referral into the program. If the GP finds the program to be suitable for you, the GP will then complete the Referral Form and send it back to us.
Our Care Coordinator will then work with your GP to coordinate initial appointments with the specialist nurse, or Allied Health Practitioners identified on the referral.
The Allied Health Practitioners are located in different sites across the Southern Yorke Peninsula.
