715 General Practice Support

Support and mentoring for general practices to increase the uptake of 715 Aboriginal Health Checks

Are you seeking to improve the capacity of your general practice to provide culturally safe care?

Sonder’s 715 General Practice Support program offers cultural support and mentoring to general practices in order to promote an increased uptake of the MBS Item 715 - Aboriginal Health Check.

As part of the program, a dedicated Aboriginal Health Practitioner will be based at your general practice and work closely with your practice staff for 1-2 days per week, providing assistance to better understand and respond to the needs of Aboriginal and/or Torres Strait Islander patients.

Is my practice eligible?

To be eligible for this program, your general practice must:

  • Be PIP IHI registered;
  • Be located in the local government areas of Onkaparinga, Playford, Port Adelaide or Salisbury;
  • Be willing to provide induction to the practice and relevant training on software to the Sonder AHP;
  • Have a number of Aboriginal and/or Torres Strait Islander clients;
  • Be willing to share specific data to support the program including the number of MBS item 715 claimed in the previous year;
  • Have availability of space and;
  • Facilitate practice staff to complete Cultural Awareness or Safety Training.

Who are the staff?

Our 715 General Practice Support staff are Aboriginal Health Practitioners and Aboriginal Health Workers.

Our team have vast experience in supporting and facilitating 715 Aboriginal Health Checks.

 

How do I access the service?

Sonder’s 715 General Practice Support program is available to practices located in Onkaparinga, Playford, Port Adelaide and Salisbury.
