Support and mentoring for general practices to increase the uptake of Aboriginal and/or Torres Strait Islander Health Checks (MBS Item 715)

Are you seeking to improve the capacity of your general practice to provide culturally safe care to Aboriginal and Torres Strait Islander patients?

Sonder’s 715 General Practice Support program offers cultural support and mentoring to general practices to promote increased uptake of the MBS Item 715 Health Check.

As part of the program, an Aboriginal Health Practitioner will be based at your general practice for up to 2 days per week. Working closely with your staff, the Aboriginal Health Practitioner will support patients from all age groups at your practice to complete their Aboriginal Health Checks.

Is my practice eligible?

To be eligible for this program, your general practice must:

  • Be PIP IHI registered;
  • Be located in the local government areas of Onkaparinga, Playford, Port Adelaide or Salisbury;
  • Have a number of Aboriginal and/or Torres Strait Islander patients and be willing to share specific data to support the program;
  • Be willing to provide induction to the practice and relevant training on software to the Aboriginal Health Practitioner;
  • Be able to host the Sonder staff member and;
  • Support practice staff to undertake Cultural Awareness or Safety Training.

Who are the staff?

Our 715 General Practice Support staff are Aboriginal Health Practitioners and Aboriginal Clinical Health Workers.

Our team have vast experience in supporting and facilitating Aboriginal Health Checks in a culturally appropriate manner.
